Katie Herb Scholarship
AWARD
The Katie Herb Scholarship will be awarded to one student every four years in the amount of $1,500. The next scholarship will be awarded beginning the fall 2013 acedemic school year.
The Katie Herb Scholarships provide financial assistance to students who are living with a congenital heart defect based on a student’s demonstrated academic achievement, activities and community involvement. The scholarships are made possible through the generous contributions from our supporters and are administered by the Katie Herb Foundation.

ELIGIBILITY CRITERIA
• You must be a student enrolling or enrolled in an accredited, four year college or university; beginning fall semester 2013
• Full-time student with at least 12 credits per semester
• Cumulative high school or college GPA at least 2.5 (verified via transcripts)
• Be a legal U.S. resident
• Family members of the Katie Herb Foundation Board of Directors are not eligible.
• Submit a complete application and all required documentation by the deadline

SELECTION PROCESS & TIMEFRAME
Application review and selection of recipients will be completed by the Katie Herb Foundation Board of Directors. Applications must be submitted in full and submitted to the Katie Herb Foundation by April 15th. All applicants will receive notification by mail no later than May 15th.
